How to Create a New Custom Form

Steps

  1. Go to Custom Forms from the sidebar
  2. Click Create New Form
  3. Configure the basic settings:
    • Form name
    • Booking types (standard, maintenance, or all)
    • Equipment scope (specific or all equipment)
  4. Set behaviour options:
    • Allow users to edit responses
    • Set form as optional or required
  5. Choose when the form must be completed:
    • During booking creation
    • Once per user per equipment
    • After booking (within a time window)
  6. Build your form using available fields:
    • Text (short/long)
    • Numbers (with validation)
    • Dates and times
    • Dropdowns, checkboxes, radio buttons
    • File/image uploads
    • Headings and helper text
  7. Save the form

Important

  • New forms are inactive by default
  • Activate the form when ready so it appears in bookings
