How to Create a New Custom Form
Steps
- Go to Custom Forms from the sidebar
- Click Create New Form
- Configure the basic settings:
- Form name
- Booking types (standard, maintenance, or all)
- Equipment scope (specific or all equipment)
- Set behaviour options:
- Allow users to edit responses
- Set form as optional or required
- Choose when the form must be completed:
- During booking creation
- Once per user per equipment
- After booking (within a time window)
- Build your form using available fields:
- Text (short/long)
- Numbers (with validation)
- Dates and times
- Dropdowns, checkboxes, radio buttons
- File/image uploads
- Headings and helper text
- Save the form
Important
- New forms are inactive by default
- Activate the form when ready so it appears in bookings
